WE ALSO RESPECTFULLY REQUEST THAT IF YOU ARE ILL WITH FEVER OR COUGH, PLEASE RESCHEDULE YOUR APPOINTMENT. IF YOU ARE ACCOMPANYING A PATIENT WHO  DOES NOT REQUIRE ASSISTANCE FOR THEIR VISIT,  PLEASE WAIT IN YOUR CAR OR OUR FOYER OR LOBBY IN ORDER TO PRACTICE SOCIAL DISTANCING FOR THE PROTECTION OF ALL PARTIES.  THANK YOU.

Please plan on arriving 15 minutes early for your first visit.  You will be required to fill out patient registration, patient history, and review of systems forms if you have not completed these forms online. This information will help your doctor to evaluate your current health as it relates to your eye care. We update this information annually to ensure we have accurate records.

Please bring your insurance identification cards and any medications you are taking. If you have been under the care of another doctor, we may ask your permission to obtain a copy or transfer some of your records.

Billing and Insurance

Ophthalmology Associates accepts many health insurance plans.

Managed care plans have their own requirement to ensure coverage and/or maximize benefits. Please consult with your insurance to see what referrals or prior authorizations are required from your regular provider before you can see a specialist.

If you do not have insurance, we ask that you pay for services at the time of your visit. If you require medical services and want to discuss the payment options please call 817-332-2020.

Minors

Permission from a parent or guardian is required before treatment may be provided to those under the age of 18 or non-emancipated minors. A consent for treatment of the minor will be obtained prior to services and the parent and/or legal guardian can direct care.

Referrals and Authorizations

Some medical insurance companies require a referral from a patient’s primary care physician before they will authorize care from a specialist. At other times to obtain maximum benefits, you may be required to obtain a referral. Any questions regarding your specific coverage, including referrals and authorizations, should be directed to your insurance company or your company benefits administrator.

Most vision insurance will require a prior authorization for service. Our office will obtain the authorization for you. Co-payment or co-insurance is due at the time of service. You will want to verify eligibility for a routine eye exam with your vision insurance prior to scheduling your eye exam. Some plans limited coverage based on the diagnosis, the provider or time since the last service.

Any questions regarding your specific coverage, including referrals and authorizations, should be directed to your insurance company or your company benefits administrator. You are responsible for any amount not covered by your plan.

Ophthalmology Associates participates with many area insurance plans, and we will be happy to submit your insurance claims for you. In order for us to bill accurately, we will need copies of your insurance cards that include identification numbers, groups numbers, and claim address and phone number. Please inform us if you change insurance coverage to ensure accurate billing.

Refund Policy

If Ophthalmology Associates owes you a refund due to an overpayment or credit balance, we will issue a refund after our billing team has verified it. Provided that there are no other balances or pending charges, we will then either refund your credit card or mail you a check depending on how you made your initial payment.

Hospital Affiliations

Some surgical procedures are performed in the office. For other surgeries recommended by your physician and agreed to by you and your insurance company, one of several local hospitals or outpatient surgery facilities will be used with the assistance of specialized surgical staff.
