Your health insurance contract is between you and your insurance company. Any questions regarding your specific coverage, including referrals and authorizations, should be directed to your insurance company or your company benefits administrator. You are responsible for any amount not covered by your plan. Payment of co-payments, deductibles and uninsured amounts are required at the time of service.
